In order to expedite your visit, please bring the following items: · A list of current medications. · Insurance card(s) and a Photo ID.Established patients should arrive 15 minutes before the appointment time.
All appointments are subject to Insurance verification. Insurances listed below are for verification purposes and do not guarantee provider participation. Please call our office if you have questions.
Chippenham Family Medicine does not prescribe narcotic medications for chronic pain. We do participate in the Virginia Prescription Monitoring program.
